M-Powered will assist in the management of complex patients in conjunction with their treating GP. Assisting GP's and Nursing Homes with Advanced Care Planning, the development and management of comprehensive Medicare compliant care plans, and the implementation of proven risk reduction strategies for their patients and practices.

We are a Private Practice there are no Medicare refunds at this stage.
